Concurrent Enrollment Form
This Parkland College Concurrent Enrollment form is for full-time University of Illinois students who wish to a course at Parkland College.  The student must be registered for the term in which s/he would like to take the course concurrently at Parkland.  It is the student's responsibility to ensure that the course is transferable to the University of Illinois.  The college signature may be obtained at 104 Mumford Hall.
 Concurrent Second Degree
This form is for students enrolled in other colleges on the University of Illinois campus wishing to complete major requirements for receiving an College of Agricultural, Consumer and Environmental Sciences degree. The form needs to be completed in full and submitted for review to the Academic Programs office. It is YOUR responsibility to obtain and submit with this form any documentation that supports the request. This form must be submitted to Academic Programs/104 Mumford Hall no later than 5:00 p.m. of the first day of classes one calendar year prior to the term of your graduation.
 
Continuing Student Status
Required if you:
  • have completed a Bachelor of Science Degree at the University of Illinois at Urbana Champaign in the College of Agricultural, Consumer and Environmental Sciences and
  • are requesting to continue to take additional course work the subsequent semester immediate following the semester of graduation.

Credit/No-Credit
The credit/no-credit grading option is designed to encourage student exploration into areas of academic interest that they might otherwise avoid for fear of poor grades.
 
Drop a Course after the Deadline
This petition is required if you request to drop a course after the fourth week deadline for eight week courses (first half-and second half- sessions and after the eighth week deadline for sixteen week courses. In the case of extenuating circumstances, special administrative exceptions to the campus deadline may be made. The petition must include relevant documentation, as the request will not be reviewed/considered without these documents.
 
Interruption of Residency
All students who are candidates for a Bachelor of Science degree in the College of Agricultural, Consumer and Environmental Sciences must complete the University of Illinois at Urbana-Champaign residency requirements as described in the Student Code. Furthermore, all students who are candidates for a degree in the College of Agricultural, Consumer and Environmental Sciences must complete at least 15 semester hours of major and option courses offered by the College of Agricultural, Consumer and Environmental Sciences in residence on the Urbana-Champaign campus and be enrolled in the College of Agricultural, Consumer and Environmental Sciences for at least two semesters.

In addition to the above residency requirement, a transfer student to this campus from a four-year college must also complete his or her senior year (not less than 30 semester hours) in residence at the University of Illinois. A transfer student from a community college must complete at least 60 hours at a senior college (4-year, degree granting institution) and the last 30 hours at the University of Illinois. All transfer students must complete their last 30 semester hours, uninterrupted, in residence at the University of Illinois.

Please Note: Effective for students entering FA06: 60 hours completed at the University of Illinois of which 21 hours must be at the 300 or 400 level.

Overload
  • Fall/Spring request for enrollment in more than 18 hours; or
  • Summer request for enrollment in more than 9 hours.

Re-Entry
This form is to be used for request to re-enter if you have only been away from the University for one semester and plan to re-enter into the same concentration. You must have left the University in good academic standing.
 
Re-Entry (Long Form)
Required if you:
  • left the University on drop status;
  • are applying to a different academic program;
  • were placed on ‘must petition’ status by their college; or
  • withdrew after the twelfth week of classes

Re-Evaluate Drop Status
This petition can only be filed by students who were placed on drop status at the end of the previous semester.
 
Re-Entry: Non-Degree Status
Required if you:
  • have completed a Bachelor of Science Degree at the University of Illinois at Urbana-Champaign and
  • are requesting to take additional course work (not the semester immediately following graduation); or have previously been classified as non-degree and desire to take additional coursework in the College of Agricultural, Consumer and Environmental Sciences at non-degree status.

Re-Entry for a Second Degree
Required if you:
  • have completed a Bachelor of Science/Arts Degree outside the College of Agricultural, Consumer and Environmental Sciences at the University of Illinois and
  • are seeking a second degree in a College of Agricultural, Consumer and Environmental Sciences B.S. program.

Special Request
This petition form is to be used for requests that do not fall under these categories: substitution of a required course; dropping a course after the campus deadline; and any type of re-entry request.
 
Substitution of a Required Course
Petitions are to be submitted for all requests of substitutions for required courses. The College of Agricultural, Consumer and Environmental Sciences does not waive total hours required in a degree program. Requests to make a substitution for a required course should be made prior to enrolling in the alternative course; having completed the alternative course does not assure approval.
